Lifeline Home Care developed a step-by-step process for potential employees to follow. These steps must be completed entirely before our team will review your documentation.

3. View and sign the job description and code of conduct forms.

4. Share your skill training and experience with us via our online form.

5. Complete our quiz to ensure you understand proper protocol.

6. Fill out our memo and client communication form.

7. Complete the W-4 Form and the I-9 Form. Please fill out each form, save as a PDF, and send it to our office either via the upload form online or by mailing it to our office.

8. Finish the process by completing the online checklist. Once Lifeline receives this final form, we will start reviewing your documentation.

NOTE: In order for us to know that all steps are completed and your application is ready to review, you will need to check that you completed each of these items through our online checklist form. No application documents will be reviewed until we receive that complete online checklist.
