Lifeline Home Care is dedicated to helping seniors stay in their homes longer, living with dignity and independence. We hire Certified Nursing Assistants (CNA), Home Health Aides (HHA), Home Companions and Personal Care Aides to provide non-medical home care services for our clients.

Filling out a job application is simple.

Lifeline Home Care developed an eight step process for potential employees to follow. These steps must be completed entirely before our team will review your documentation.

Starting with step 2, you will receive the link for the next step once you’ve submitted the form. The link will be given in the confirmation screen.

In order for us to know that all steps are completed and your application is ready to review, you will need to check that you completed each of these items through our online checklist form (step 8). No application documents will be reviewed until we receive that completed online checklist.

  • Apply for the position via our online application form (step 1).

    • Step 1 is done in a system outside of our website. You will be taken to a different webpage to complete it. Once Step 1 is completed, return to this Employment page on our website to start Step 2 (link below). All other steps will be completed on the Lifeline website. Only after you have completed step 1 can you move onto step 2.

  • Complete the credentials and identification form (step 2). You will need documentation of each category requested in the form. We recommend you review the form ahead of time so you know which documents you will need to have on hand.

  • View and sign the job description and code of conduct forms (step 3).

  • Share your skill training and experience (step 4) with us via our online form.

  • Complete our quiz (step 5) to ensure you understand proper protocol.

  • Fill out our memo and client communication form (step 6).

  • Complete the W-4 Form and the I-9 Form. Please fill out each form, save as a PDF, and send it to our office either via the upload form online (step 7) or by mailing it to our office.

  • Finish the process by completing the online checklist (step 8). Once Lifeline receives this final form, we will start reviewing your documentation.
